As they all are, looks can be deceiving. My wife had two back surgeries when there for both, had her stay, returned home, she had trouble with the upper part of back, went back surgery, went back to divine, close to end of yr, Divine told us we we're out of days to stay there, no real notice for us to locate a skilled care facility, no way for me or family to care for her, she couldn't even stand 20 sec, or hold her self up, we appealed for her to stay, Commence Heath ( formally Livanta LLC) came back with a determination letter, they denied it, on Divines (PT dept) stating that my wife walk up to 50 ft, perfom bed transfer, chair with assistance, they lied, there as no way she had that ability. The letter from this company, parts of looks like the copy pasted the notes form Divine, must have been previous stays at the place. Don't trust there PT dept. Look else where. Stay away.

My mom was cared for here for 30days. I have nothing but good things to say about the staff here. In particular Luis, McKenzie, and like 4 other nurses whos names slip my mind who were extremely caring and compassionate to my mother. One day i came to visit her and found her with her hair done! My mom is non verbal, bed ridden,at the time on a ventilator. They made sure she didnt sit in her mess for too long. They treated her with the upmost respect and compassion and successfully wheened her off the ventilator.I was actually sad to transfer her to the next facility. Even the maintenance staff was kind and compassionate. Michelle came in to clean my moms room one day and we had such a great conversation. She shared her experiences with her father and i have to say, the conversation uplifted me. With all the uncertainty of my moms situation hearing from someone with a similar experience was uplifting. I want to thank not just the people mentioned but everyone involved in my mother's care. 🙏🏽🙏🏽🙏🏽🙏🏽

I would not consider putting my loved one in this facility. My mother was here a few months ago, and a lot of things occurred, my daughter is here on July 20th. She was admitted. I observed the nurse just walking in the room. Not announcing herself and putting her hands-on my daughter, so I walked in the room behind the nurse. Yes, I was on the phone, but I'm not the patient my daughter is. Mind you, it's going on 8:45shift changed at 7 AM. My daughter was wet, I was trying to find someone to change her, someone came in and answered the call light. Instead of helping they'll get her nurse to tell her that she was wet. I waited like 30 minutes after that. No one came. My daughter was neglected and there was nothing I can do about it. There was no supervisors to get in touch with. There was nothing that could be done over the weekend. The nurse misused security because she knew she was wrong and there was nothing I can do about it. I was asked to leave for the rest of the day. Knowing that i'm the main primary for my daughter, but that didn't matter, I was asked to leave

There is absolutely nothing negative. I can say about the hospital. And the care that you will receive when you're here from day one I felt like i belonged here. I wasn't nervous in any kind of way. The nursing staff will all this staff period is wonderful. And if I had my choice to any hospital that I needed to go to in the future. I almost definitely. Would with choose to come to osf Healthcare mercy continuing care hospital. That you the all the staff. You all had treated me with kindness and respect, spas well as made me feel like I was a part of the OSF family and that I wasn't going through any of these treatments alone.

Highly recommend this place. I had a severe MCA stroke and spent 6 weeks in the hospital before transitioning here, andwas here from march 7 until March 20. The rooms are nice and the staff makes you feel like family. Everyone was very friendly and accommodating. My occupational and physical therapists were both great. Aaron helped me become stronger and worked with me on the specific areas i needed. Kaitlynn my occupational therapist was amazing too. She was so sweet and braided my hair for me everytime i had a session with her. I miss the people there but i am walking everyday and staying active and healthy so i don't plan on returning lol. I'll admit i didn't want to go to rehab but I'm so glad i did because they helped me become stronger and the person i was before my stroke. Would definitely recommend this place to anyone who needs recovery and therapy

I was in here from 8/21/2024 to 9/10/2024. Dr Savita S Reddy was awesome and very caring and always addressed my concerns. My physical therapist Yul was awesome every day. I was weak in my right leg and left arm. Yul was caring and pushed me to get stronger. Brad my occupational therapist was excellent too. All the nurses were caring and supportive too. The food was also very good. This is how a hospital should be ran. This place is a 5 and the OSF ER is awful. OSF Transitional Hospital 5 stars ER 0 stars and the hospital itself after admitting was 4 stars. Also all my meds were on time as well

The nurses, therapists, doctor and case managers were professional and caring. Each were a beautiful example of caregivers during a very difficult time for our family. My father needed tremendous care which they provided to the highest level with a smile on their faces. Every question we had was answered thoughtfully. We couldn't have asked for a better team to help care for my dad as his body and mind fought cancer.
