We're sorry this was a frustrating experience, especially during an already difficult time, and that we couldn't provide more support as you navigated the process. We strive to return calls within one business day, and we sincerely apologize for any delay. Our goal is to guide every prospective resident and family through the process as clearly and compassionately as possible. We recognize that we fell short of our goal, and we take full responsibility. We'd welcome the chance to speak with you directly and hear more about your experience and ways our team can improve. Please don't hesitate to contact the Administrator, Rodger Groves, at 714-256-8008.

I called the admissions director and she remembered us immediately and treated me like a family member when I told her we needed to come back. Everyone was so kind and my mother was happy to see familiar faces. The CNAs are all so wonderful and brought my mom back to life. The dietary department was incredibly thoughtful and even though they knew her from before, they still met with her again to ensure her tastes didn’t change. Honestly, if you had your parent go anywhere, this is the place. Housekeeping guys were friendly and kept the room clean, laundry staff were caring when putting my moms things away, reception was welcoming. I can’t say enough about this building. I’m so glad my mom is back home, but Park vista is the reason for it.

The CNAs at Park Vista have been amazing, and I truly appreciate how caring and attentive they have been toward my elderly father. Five stars to the CNAs.
Unfortunately, my overall experience with Park Vista has been very disappointing. The facility looks impressive on the surface, but our experience with the rehabilitation, nursing, medical, and radiology services has been very different.
My father was transferred to Park Vista directly after being treated for aspiration pneumonia at UCI Orange and Placentia Linda hospitals. During approximately 13 days between the two hospitals, he had seven chest X-rays, and neither hospital suspected TB.
At Park Vista, an outside radiologist interpreted a chest X-ray as suspicious for TB. My concern is that his extensive recent imaging and medical history were not adequately considered or compared before this conclusion was made. As a result, my father was placed in isolation, and his PT and OT were stopped—especially concerning for an 89-year-old who was already significantly weakened and desperately needed rehabilitation.
The subsequent QuantiFERON-TB test was indeterminate—not positive. The mitogen level is an important part of the QuantiFERON test because it serves as a positive control to show whether the patient's immune system is capable of mounting an adequate response. When the mitogen response is too low, the test is reported as indeterminate, meaning it cannot reliably determine whether TB infection is present; it does not mean the result is positive. A low mitogen level can occur when someone is elderly, immunocompromised, or recently seriously ill, as my father was. I was disappointed that this distinction and the explanation for the indeterminate result did not seem to be fully understood or appropriately considered when decisions were being made about his care.
Thankfully, my father did not have TB.
What is most frustrating is that my father did not make meaningful improvement during his time at this facility. As his children, we took turns coming in and doing everything we could to help him—even if we could only make small improvements ourselves. We were constantly trying to advocate for him and keep him moving forward.
Even wound care was frustrating. He was scheduled to receive wound care on Tuesdays and Thursdays, yet on those scheduled days, no one came until around 8:30 PM. That is not the level of timely care I expected from a skilled nursing facility.
I completely understand the need for infection-control precautions when TB is reasonably suspected. However, precautions should also be based on the patient's complete clinical history, recent imaging, and appropriate interpretation of testing. In this situation, the isolation and cancellation of rehabilitation felt excessive and unnecessarily disruptive. It also resulted in considerable use of PPE and healthcare resources.
My overall impression is that Park Vista looks good on the surface, but our family's experience revealed significant problems with communication, clinical coordination, rehabilitation, and medical decision-making.
Five stars for the CNAs. Unfortunately, based on our experience, I would give zero stars to the rest of the team involved in my father's care, including the radiology service responsible for the TB interpretation.
I hope Park Vista takes this feedback seriously. Elderly patients recovering from serious illnesses need not only infection-control protection, but also competent rehabilitation, timely nursing care, appropriate medical judgment, continuity of treatment, and—most importantly—someone willing to advocate for the whole patient rather than simply following one report without considering the bigger picture.
My family did everything we could to advocate for my father. I wish we had felt that the entire care team was doing the same.

Park Vista Health Center provided outstanding care for my mother. My mom has been to Park Vista twice now for over 4 months total. The nurses and CNA's are attentive, kind and committed to their patients' well-being. They were very good with the restrictions she had & keeping up with her medications. Additionally, the physical and occupational therapists were absolutely phenomenal. They were patient, encouraging, and tailored the rehabilitation specifically to her pace, helping her regain her independence. Roger made sure her TV was working each night so that she wouldn't miss a Dodger game. The facility is very well maintained. The building is always clean, bright and comfortable. I spent a lot of time there and felt very welcomed by the staff. We felt at ease when we left her for the night knowing she was being taken care of. Thank you to all the staff for helping in my mothers recovery!!

Dannette,
Thank you so much for taking the time to share your experience. We are truly honored that your mother chose Park Vista not just once, but twice during her recovery journey. That trust means the world to us.
We are delighted to hear that our nursing team, CNAs, therapists, and staff helped make her stay comfortable, supportive, and successful. Your kind words about the care, encouragement, and attention she received will mean a great deal to the team, and I will be sure to share your feedback with them.
Thank you again for allowing us to be part of your mother’s recovery. We wish her continued health and happiness, and we are grateful for the opportunity to care for your family.

I was warned often that they rarely have beds, but if I call and tour, I had a higher chance of getting my friend accepted. They were right. My friend recovered there for a few weeks and throughout her stay she would tell me- she wished she could stay. She enjoyed therapy, food, and nursing. When I would visit, I would see how the staff interacted with others, and it made me feel happy knowing when I leave, my friend was cared for. I would recommend this health site.

I had a really frustrating experience with Park Vista Health Center, mainly with rehab.
What I didn’t realize at first is that the therapy isn’t actually run by the facility. It’s handled by a company called Select Rehabilitation that comes in separately, and that disconnect really shows. As a family member, that was confusing because you assume everything is coordinated by the place your loved one is staying at.
My mom was taken off therapy because she was told she “wasn’t making progress,” but I saw other patients in much worse condition still getting therapy for long periods of time with no real change. One patient a few rooms down had been there over a year with no movement or communication and was still on services. That didn’t make sense and raised real questions about how those decisions are being made.
The therapy itself felt very basic. Mostly passive range of motion, not much active work, and not much visible effort toward real improvement.
The biggest issue was communication. I asked multiple times for clear explanations during care plan family meetings and when I spoke to staff, but I kept getting redirected or given vague answers that never explained why decisions were being made or why therapy was stopped.
After looking into Select Rehabilitation, I also saw mentions from other families of concerns and even some legal disputes in situations involving how therapy is handled in facilities they work with. It made our experience feel more familiar, especially given the lack of clear communication we ran into.
It also makes me question why a company like Select Rehabilitation would be the contracted therapy provider here when there are other rehab companies with strong reputations. It raises questions about why this setup was chosen if it creates this kind of disconnect in communication and accountability between families, therapists, and the facility.
Overall, Park Vista looks like a nice facility when you first walk in, but the rehab experience didn’t match that impression. When recovery is the main reason someone is there, that matters a lot.
If you have a family member here, stay very involved, ask a lot of questions, and don’t assume everything is being clearly explained

Paul, thank you for sharing your experience. I’m sorry to hear how frustrating this felt—especially when communication wasn’t as clear as it should have been. Care plans can vary significantly based on each resident’s needs. While some individuals are here for short-term rehab, others receive ongoing support through our restorative nursing program focused on maintaining mobility and function. I would appreciate the opportunity to speak with you directly to better understand your mom’s experience, answer any questions, and provide additional clarity. Please feel free to contact the administrator, Rodger Groves, at 714-256-1000.
Thank you again for your feedback—it’s important to us.
